Terms of Service

Thank you for choosing R. Bart Sangal, MD at Sleep and Obesity Medicine (“we,” “us,” or “our”). These Terms of Service (“Terms”) apply to all patients scheduling appointments through sleepandobesitymed.salusemr.com.

1. Acceptance of Terms

By scheduling an appointment, you agree to be bound by these Terms. If you do not agree, please refrain from booking an appointment.

2. Services Provided

We offer consultations, evaluations, treatments] for sleep disorders and for overweight/obesity. Services are provided based on availability and individual patient needs.

3. Appointment Booking

• Accuracy: You must provide accurate and complete information when booking an appointment.
• Confirmation: Appointments are subject to confirmation. We will contact you to confirm details.

4. Cancellations and Rescheduling

• Policy: Cancellations or rescheduling must be made at least 1 business day (24 hours not counting weekends and holidays) in advance.
• Fees: A cancellation fee will apply for missed appointments or cancellations (100% for less than 1 business day, 50% for 1 business day to 1 business week, 20% for greater than 1 business week).

5. Payment

• Fees: Payment is due at the time of booking an appointment unless other arrangements have been made.
• Insurance: We do not accept any insurers. It’s your responsibility to make payment for services. You may submit our invoice to your insurer (other than Medicare) for possible reimbursement.

6. Privacy and Confidentiality

We are committed to protecting your privacy. Your information is handled in accordance with our Privacy Policy (published on our website, www.sleepandobesitymed.com, and applicable laws.

7. Patient Responsibilities

• Preparation: Follow any pre-appointment instructions provided.
• Honesty: Provide complete and truthful medical histories and information.

8. Limitation of Liability

Our liability is limited to the extent permitted by law. We provide no warranties, express or implied, regarding your treatment outcomes.

9. Changes to Terms

We reserve the right to amend these Terms at any time. Any changes will be communicated to you when applicable.

10. Governing Law

These Terms are governed by the laws of Michigan, without regard to conflicts of law principles.
